Kurz gesagt
Download Staht's free anchor site test report as a fillable PDF. It follows what BS 8539 recommends a report records: the job, the anchor and the test asked for, the base material, the installation, each anchor's position and result, the equipment and its calibration, and a sign-off saying the report records facts only.

Is it a certificate or a report?
People search for an "anchor pull test certificate", and the document is the same thing: a record of the tests. We call it a report because it doesn't certify that a fixing is fit for use. It states what each anchor did on the day; the specifier or responsible engineer decides what that means.
How to fill it in
- Fill in parts 1 to 3 before the visit, from the test request.
- Fill in part 4 only if you installed the anchors or watched them go in.
- Give each anchor's position on a drawing, or describe it well enough for someone on site to find it.
- Name any failure mode using the groups in first movement and failure modes.
- Sign, and attach the method statement and the tester's calibration certificate.
- Give the report to the person who asked for the tests.
Do I need it if I use Staht Connect?
Not for the results: the Staht Connect app records each test's peak load, target load, pass or fail and time, with photos and a map pin, and the dashboard makes a report in one click. Check that whatever produces your report covers every item in the template: use the app's custom form fields, or this template, for items such as the drill bit diameter.

Yes. Part 2 records the test type and procedure, and the results table has a column for the load at first movement, which BS 8539 asks to be recorded in site tests.
It is one of the items BS 8539 lists for a site test report, alongside the hole diameter and depth.
