Kort sagt
Download Staht's free scaffold tie anchor test record: a fillable PDF with a results sheet for 20 anchors and eight cut-out tags. It records every field in TG4:25's typical anchor test report (its Appendix A), and more, and the tags carry the test date and load, which TG4:25 says it is common practice to put on each proof-tested anchor.

Key figures
| Format | Fillable PDF, A4, two pages: type on screen or print |
|---|---|
| Results sheet | 20 anchors |
| Tags | 8 cut-out tags: date, test load, result, tester |
What's in the template?
| Part | Fields |
|---|---|
| The job | Site, date, record number, client, scaffold contractor, anchor type and size, base material, tie duty |
| The test | Preliminary or proof; the minimum test load; who set it |
| Results | For each anchor: test number, location (elevation, lift, bay), minimum test load, load reached, pass or fail, tagged, notes |
| Tester | Name, company and signature; tester make, model, serial number, last calibration date and calibrated accuracy |
| Tags | Date, test load, result and tester, and a box to mark a failed anchor, on each of 8 tags |
What minimum test load goes on the record?
For a proof test, 1.25 times the tie duty from the TG20 or TG30 compliance sheet or the design. For a preliminary test, twice the tie duty, or three times for nylon anchors unless the maker states otherwise. See scaffold tie test loads for each tie class.
How are the tags used?
TG4:25 notes that tagging each proof-tested anchor with the date and load is common practice. Fill in a tag and fix it to the anchor or tie, so the next inspector can see it was tested, when and to what load. For an anchor that failed, mark the tag's failed box with an X, so nobody uses it.
Is this a scaffold tie test certificate?
It's the record a certificate would hold: what was tested, to what load, and the result. It doesn't approve the anchors or the scaffold design; it states what happened on the day.
Can the Staht Connect app replace it?
For the results, yes: the t25 pairs with Staht Connect, which records each test with the peak load, target load, pass or fail, a photo and a map pin. You still tag the anchors on site.
